What Is an Annular Tear?

An Annular Tear is a small split in the annulus, the tough outer ring of a spinal disc. The split may cause local back or neck pain. It can also allow inner disc material to press outward. An annular tear may appear with disc wear or after a sudden strain.

Key Takeaways About Annular Tear

  • The tear affects the disc’s outer ring.
  • Pain may stay near the spine or spread outward.
  • An annular tear is not the same as a herniated disc.

How Annular Tear Works

A medical diagram card titled “ANNULAR TEAR” shows a cracked ring model beside a magnifying glass and pen on a desk

An annular tear affects the annulus, which surrounds and supports the softer center of a spinal disc. The tear may irritate nearby tissues and cause pain near the spine. Some tears also let the disc center push toward the opening.

Related glossary terms: Disc Bulge, Disc Degeneration, Lumbar Radiculopathy.

A clinician considers symptoms, movement, strength, sensation, and reflexes during an exam. Imaging may show changes in the disc, but an image does not always explain every symptom. A tear can occur in the neck or lower back, depending on the affected disc.

Related Disc Changes

  • A disc bulge pushes outward without a clear break in the outer ring.
  • A herniated disc involves inner material moving through a weakened or torn area.

From the Advanced Injury Care Clinic team

When we assess an annular tear during Herniated Disc Treatment, we connect the reported symptoms with exam findings. We also check for signs of nerve involvement and consider how movement changes the pain. Our team uses that information to guide a focused care plan.

Annular Tear in Practice: A Real-World Example

A person may develop lower back pain after lifting and twisting. An exam may suggest disc irritation, while imaging can show an annular tear without proving that the tear causes every symptom.
