What Is Forward Head Posture?

Forward Head Posture is a postural pattern in which the head rests ahead of the shoulders when viewed from the side. The position can increase demand on the muscles and joints of the neck and upper back. Prolonged screen use, seated work, and limited postural awareness may contribute to this pattern.

  • Forward Head Posture is not the same as neck pain.

How Forward Head Posture Works

A hand rests on a seated man's shoulder in a bright room with a blurred computer monitor and window behind him

Forward Head Posture changes the alignment between the head, neck, shoulders, and upper back. As the head moves forward, neck muscles may work harder to support its position. The upper back and shoulder muscles may also adjust their activity to maintain balance.

A clinician may observe the person from the side during a posture assessment. The assessment considers the head position alongside shoulder height, upper-back shape, neck movement, and muscle control. A person may show this pattern while standing but not during movement, so static posture does not describe every part of function. Postural correction may address movement habits, joint mobility, and muscle coordination together.

From the Advanced Injury Care Clinic team

When we assess Forward Head Posture, we look at head and shoulder alignment during standing and movement. We also consider neck motion, upper-back mobility, and daily activities that may reinforce the pattern. Our team uses these findings to guide Postural Correction in a way that matches the person’s movement needs.

Forward Head Posture in Practice: A Real-World Example

A person who works at a laptop may gradually bring the head toward the screen while the shoulders round forward. During Postural Correction, the clinician may observe that seated pattern and then check whether the person can regain a more balanced position while standing and moving.
