Functional Medicine Consultation for Forest Hills Health Planning

A functional medicine consultation gives patients space to explain symptoms and health concerns in context. The discussion can include medical history, current medications, accident-related symptoms, and questions about diagnostic needs.

This page serves patients who want to prepare well before meeting the care team. The key decision is what information to bring and which concern deserves the most attention first. A consultation isn't a substitute for urgent medical care when symptoms need immediate attention.

Patients in Forest Hills can also discuss chiropractic treatment when neck pain, back pain, whiplash, or other musculoskeletal concerns are part of the picture. Our team uses the consultation to explain care choices in plain language.

  • Write down the main symptoms and when they began.
  • Bring available medical records and test results.
  • List current medications, supplements, and recent changes.
  • Note questions about diagnosis, treatment, and next steps.

Dr. Dar’Neshia Roberts and her staff provide top-tier customer service and are super personable, knowledgeable, and always willing to further educate their patients if needed. The team works quickly and efficiently without making you feel rushed, and the clinic is very clean and always smells fresh. While I’m not happy that I got hurt, I’m extremely blessed to have found AIC. This clinic is warm, welcoming, and inclusive, and I truly felt comfortable and respected as a person of color. I highly recommend their services!!!

This place is amazing. The staff is friendly. The office is clean and laid back. Not much of a wait. The doctor walks you through every step and tells you what he’s going to do and has answers any questions I’ve had to the best of his knowledge. Normally I am afraid to go to doctors and be in offices but this place has been a different experience. I would recommend anyone to come here. The front desk ladies are helpful and friendly

Questions About Preparing for Functional Medicine Consultation

How should I prepare for a functional medicine consultation?

Health goals come first, so prepare a short list of your main concerns and questions. Bring medical records, a symptom journal, and a medication list when available. Note when symptoms began and what changes them. Advanced Injury Care Clinic uses this information to discuss diagnostic needs and treatment options in clear language.

What medical records should I bring to the appointment?

Medical records should include prior evaluations, imaging information, diagnoses, and treatment details related to your concern. Bring what you have, even if the record is incomplete. The care team can review the available information and explain what other details may help the discussion.

Which symptoms belong in a symptom journal?

A symptom journal should track the concern, when it appears, and what makes it better or worse. Include sleep changes, activity limits, headaches, neck pain, back pain, or other symptoms tied to your main concern. Short daily notes are enough to support a useful conversation.

When should I discuss a medication change?

Medication changes should be discussed with the medical team before you alter your current plan. Bring the medication name, reason for use, and any change you noticed. Our team can explain which medication questions belong in the consultation and when another medical discussion is needed.

Why does a consultation review chiropractic care too?

Chiropractic care may matter when symptoms include whiplash, neck pain, back pain, or other musculoskeletal concerns. The review helps connect those symptoms with your health history. Our chiropractors and medical professionals explain whether coordinated care fits the concerns discussed.

Can I request this consultation after a workplace injury?

Workplace injuries can be discussed during the consultation, including symptoms, medical records, and documentation needs. Bring any reports or care details you have. Our team can help document the injury and prepare detailed reports as part of workplace-related care.
